I’m Young: Could I Still Have Low T?

Posted on June 24, 2025 by root

In recent years, the conversation around testosterone levels has broadened, moving beyond older demographics to include younger men. For many, the question arises: “I’m young—could I still have low testosterone?” As a board-certified, fellowship-trained urologist and men’s health specialist, my goal is to demystify low testosterone (often referred to as Low T) and its implications for younger men.

What is Low Testosterone?

Testosterone is a crucial hormone in the male body, primarily responsible for the development of male physical characteristics, maintaining muscle mass and regulating libido. While testosterone levels naturally decline with age, The Urology Care Foundation™ affirms that men can experience low testosterone at any age, including their teens and 20s. Low T is diagnosed when testosterone levels fall below the normal range, which is typically classified as 300 to 1,000 ng/dL.

Symptoms of Low T in Young Men

A July 3, 2020, article in the Urology Times Journal reported, “Testosterone deficiency has a prevalence of 10%-40% among adult males, and 20% among AYA men aged 15-39 years.” Many young men may experience symptoms of low testosterone without recognizing them as related to hormone levels. Common symptoms include:

  • Fatigue: A significant drop in energy levels, making it difficult to perform daily activities.
  • Depression or mood changes: Experiencing prolonged feelings of sadness or irritability.
  • Decreased libido: A noticeable decline in sexual desire.
  • Erectile dysfunction: Difficulty achieving or maintaining an erection.
  • Reduced muscle mass: Loss of strength or muscle mass that doesn’t correlate with lifestyle changes such as diet or exercise.
  • Increased body fat: Particularly around the waist, which may lead to concerns about self-image.

If you notice any combination of these symptoms, it’s essential to consult with Dr. Houman for an evaluation.

Causes of Low Testosterone in Young Men

A January 2020 article in the Frontiers in Endocrinology journal states that the causes of low testosterone can vary and may include:

  1. Genetic conditions: Disorders such as Klinefelter syndrome can impact testosterone production.
  2. Chronic illnesses: Conditions like diabetes and obesity can lower testosterone levels.
  3. Stress and psychological health: Chronic stress, depression and anxiety can adversely affect hormone levels.
  4. Environmental factors: Exposure to endocrine disruptors found in plastics, pesticides and other chemicals can influence hormone production.
  5. Lifestyle choices: Poor diet, lack of exercise and substance abuse can contribute to a decline in testosterone levels.

Understanding Your Health: When to Seek Help

Understanding your unique body chemistry is essential in recognizing when levels may be low. If you’re experiencing the symptoms mentioned earlier, it may be worthwhile to get a hormonal assessment. A blood test is the most effective way to measure testosterone levels accurately.

Once diagnosed with Low T, several treatment options are available that can help restore hormone levels effectively. Discuss these with Dr. Houman, who can help tailor a plan that meets your specific needs.

Nasal Gels – Natesto

For young professionals who are concerned about preserving their fertility while treating low testosterone, nasal gels such as Natesto are an excellent option. Natesto is one of the few treatments that maintain sperm production, making it particularly appealing for men considering starting families in the future.

Jatenzo/Kyzatrex

For those who prefer a more straightforward oral medication, Jatenzo or Kyzatrex are great choices, especially for young men with good insurance coverage.

Subcutaneous Pellets

Subcutaneous pellets may be the ideal solution for young professionals with busy schedules who prefer minimal maintenance. These small pellets are inserted under the skin and release testosterone consistently over time.
