The first mechanism involves nerve damage. When diabetes is poorly managed, elevated blood sugar levels can cause permanent nerve damage. Since these nerves are responsible for stimulating the dilation of arteries that increase blood flow to the penis during arousal, this damage directly impairs the erectile response.
The second mechanism involves vascular changes. Diabetes negatively affects arterial inflow by decreasing the lumen (internal space) of the arteries. This reduced luminal capacity results in diminished blood flow to the penis, making it difficult to achieve or maintain a satisfactory erection.
